Scope and Content | Application Form includes the following information: Personal details of Witness 48 and their legal representative, Alan Collins of Pannone Solicitors, London; Details of care referencing their time at Haut de la Garenne around 1972 and 1975, and the names of other children in care who were their friends or associates; Details physical and sexual abuse alleged to have taken place between 1972 and 1975 mentioning members of Haut de la Garenne staff; complaints of abuse to the States of Jersey Police in 1980; previous civil proceedings, referencing a claim sent to Mourant Ozannes in May 2011; and Witness 48's declaration of truthfulness signed in the presence of a solicitor on 4 April 2012.
