| Scope and Content | Refers to receipt of a report from Joe Kennedy (Manager, Residential and Secure Children's Services) on 12 April 2009 informing him of a restraint incident at Greenfields on 11 April 2009 outlining a background to the incident, action taken, considerations and the staff and residents involved. Describes visit to Greenfields on 16 April [2009], setting out details of conversations with four residents involved in the incident providing each of the resident's interpretation of the incident and their responses to specific questions regarding the restraint. Refers to a debrief conducted with [a female staff member involved in the incident] at Heathfield on 16 April 2009 and her written report of the incident. Sets out impressions of the incident, noting inconsistencies in the four reports from the residents, comments about the CCTV evidence contradicting accounts of residents regarding the nature of the restraint, reports of staff, difficulties associated with the management of four residents compared to just one resident, remarks about Joe Kennedy and general comments about policy on physical restraints.
