Scope and Content • Notes of a meeting on 9 November 2000 at Maison Le Pape regarding an incident involving [a member of staff and a resident at Heathfield] on 2 September 2000. Lists Dalores Davy as amongst those present at the meeting. Outlines the circumstances of the meeting, referring to a violent incident which occurred at Heathfield on 2 September 2000 during which [the member of staff] was injured. Sets out [the member of staff's] account of events leading up to the incident and of the incident itself and sets out a number of discussion points on the matter, particularly regarding: issues regarding the use of appropriate and effective restraint in order to protect children and staff, referring to a training programme; decisions made by staff on the day of the incident; problems regarding the quality of staffing at Heathfield; follow up care shown towards [the staff member]; and practices referred to by [the staff member]. • Letter dated 19 February 2002 from Dr A N Graham-Cumming, Specialist Occupational Physician, BMI Health Services to Mr G Jennings, Chief Executive, Health and Social Services. Copy sent to Mr M Pinel. Refers to the case of [the staff member], noting her continual absence from work since 2000 following the incident when she was assault by a child in Septembe 2000. Details two issues raised by the case, relating to [the staff member] not having received training in techniques of physical restraint and a delay in referring [the staff member] for occupational health advice. • Letter dated 19 February 2002 from [Marnie Baudains], Manager, Children and Adult Services to Dr A N Graham-Cumming, Specialist Occupational Physician, BMI Health Services. Copies sent to Graham Jennings and Tony Le Sueur. Responds to concerns raised in Dr Graham-Cumming's letter to Mr Jennings dated 4 January 2002, particularly regarding [the staff member] not having received appropriate training in techniques of physical restraint and an apparent delay in referring [the staff member] to BMI. Includes comments about the quality of communication by the Children's Service to BMI on the case and in general.
