Application Form Personal details: Includes name, address and place and date of birth. Details of care: Mentions placements at Westaway Crèche, Haut de la Garenne, Norcott Villa [Family Group], and foster homes. Includes family background referencing mother's inability to cope, lack of involvement of father and court order placing Witness 43 in care. Also includes names of other children in care who were their friends or associates. Abuse alleged to have taken place: See Appendix 2.
