Application Form Includes personal details about Witness 135, including her place and year of birth. Provides details of care, mentioning residence at Haut de la Garenne from 1969 to 1974 and relevant family background mentioning residence with mother, father's desertion when Witness 135 was a baby and mother's inability to cope. Confirms to have not provided a statement to the States of Jersey Police as part of the historic abuse inquiry, noting that she did not consider providing a statement until publicity about the investigation on television and now felt she should provide her account. Declaration of truthfulness signed by Witness 135 in the presence of C R Derham, solicitor-advocate, on 5 July 2012. Supporting Documentation to the Application Form: 1. Authority for access to Medical Practitioner Records signed by Witness 135 on 28 June 2012. 2. Abuse alleged to have taken place, 1969-1971. Provides Witness 135's account of abuse suffered from 1969 to 1971 whilst in residence at Haut de la Garenne. Gives details of six or more incidents of sexual abuse by Mr [Gordon] Wateridge, mentioning the snooker table in the lounge area and men's toilets. Notes that Wateridge would advise Witness 135 not to tell anybody about the abuse as she would not be believed. Also gives details of sexual abuse by Mr [Ray] Williams frequently over a 15 month period, describing him as opportunistic. Also describes sexual abuse by Mr [Colin] Tilbrook. Details Witness 135 reporting abuse by Williams to a fellow resident at Haut de la Garenne, noting that conversation arose when the other resident disclosed abuse by Williams.
