Refers to a question raised at a meeting of the Review Panel on 24 November  regarding Witness 125's condition. Refers to Dr [Anthony] Essex-Cater and Dr [S M] Milligan having no notes relating to Witness 125's [medical condition], noting concerns expressed by them why Witness 125 had not been referred to the Paediatric Department for assessment. Describes a breakdown in communication between Dr [James] Hugh and the Public Health Department, commenting on need for contact between the two parties. Requests information concerning to Dr Hugh's recent contact with Witness 125, particularly action advised with regards to [Witness 125's condition] and possible onward referral to the Paediatric Department. Refers also to a school report on Witness 125 and assessment of Witness 125 by Mr [Jim] Hollywood produced at the Review Panel.
