1. Personal details. Provides details of Witness 145. 2. Claimaint's representative. Names representative Solicitor David Coleman of Lavelle Coleman [Solicitors], Dublin. 3. Details of Care. Refers to Care in the States of Jersey between 1974 and 1984 at Brig-Y-Don [Children's Home], Les Chênes [Residential School], and Boys Hostel Basil Lodge [Basil Lodge Hostel]. Refers to members of Witness 145's family also resident at Les Chênes, and refers to Witness 145's father's alcholism and emotional and physical abuse of Witness 145, leading to Witness 145's placement in Care. Provides the names of other children in Care with Witness 145. 4. Abuse alleged to have taken place. Provides details of abuse at Les Chênes by members of Les Chênes staff, and Mario Lundi [Lundy], as witnessed by other staff and residents at Les Chênes. Names Dr Brendan Kellet and Dr [John] Coates. Appendix 1 - Details of Abuse. Refers to Witness 145's arrival at Les Chênes, and placement in a bare cell similar to that in a Police Station for six weeks, after which Witness 145 was moved to a bedroom shared with other boys. Describes emotional and physical abuse at Les Chênes by staff towards residents including Witness 145. Refers to one of the abusive members of staff and reguarly intoxicated on duty, also mentioning an incident of physical abuse by the member of staff against a resident in the shower, and an incident where a resident allergic to tomatoes was forced to eat tomatoes. Refers to physical abuse by the member of staff reguarly intoxicated on duty in the art room. Refers to the member of staff of Les Chenes known as the 'Pinball Wizard' due to his physical abuse of residents. Mentions caning by the member of staff and Mario Lundy.
